The Effect of Eustachian Tube Function on the Postoperative Result after Tympanoplasty

Abstract
Objective: The objective of this study is to assess the value of the eustachian tube function (ETF) of otologic patient received tympanoplasty in predicting patient outcome.

Subjects and Methods: From september 2004 to August 2005, a total of 52 cases of tested by one audiologist and operated by one otorhinolarygologist at the kangnam sacred heart hospital was involved. We divided the evaluation methods of ETF into the followings£ºby 1) Modified inflation-deflation test, 2) Valsalva¡¯s maneuver, 3) Middle ear mucosa state, 4) Contralateral ETF state. All patients received ETF tests on both ears. The middle ear mucosa was examined during surgery and described as a ¡¯normal¡¯ or ¡¯abnormal¡¯. We also evaluated contralateral ETF. The postoperative result was estimated at least 3 month later. Statistical analysis was performed using SPSS version 12.0. A p-value less than 0.05 was considered significant.¡¡

Results: 86 patients were enrolled, and 52 patients (52 ears) were included in this analysis. The postoperative eardrums were good in 39 ears (75%), poor in 13 (25%). Of 13 poor outcomes, 4 ears were re-perforated and the others thickened or retracted. All evaluation methods of ETF had a significant impact on treatment response. Correlation between modified inflation-deflation test and Valsalva¡¯s maneuver, middle ear mucosa state was found, but contralateral ETF not.

Conclusion: In our study, the ETF is very important role in predicting patient outcome, but not absolutely. Modified inflation-deflation test, Valsalva¡¯s maneuver and middle ear mucosa state are good techniques in evaluating the ETF. In otologic surgery, we should consider all the factors involved and make a decision case-by-case.
